Media Release Agreement for Students Under 18 and above

​​Dear Parent/Guardian (referred to throughout this document as “I,” “You,” or “Your”):

This Media Release Form authorizes Brain Master Inc. (“Brain Master” or “we”) to capture, utilize, and distribute any media content featuring your child(ren), including but not limited to photographs, audio/video recordings, student work samples, quotes, academic progress, testimonials, or any representation of their likeness (hereinafter referred to as the “Materials”), for the stated purpose outlined below.

By signing this form, you grant Brain Master full and exclusive ownership of all rights to the materials—globally and indefinitely. This includes, without limitation, the rights to reproduce, display, modify, adapt, edit, publish, license, archive, translate, broadcast, distribute, or otherwise use the Materials in any form and by any media or technology currently existing or developed in the future. These rights will be held solely by Brain Master Inc. and protected under our intellectual property and copyright policies.

This agreement is irrevocable and permits Brain Master to proceed based on your consent. You waive any right to inspect or approve the materials before or after use and acknowledge that Brain Master and its authorized affiliates or agents hold full creative discretion. All copyrights, moral rights, and rights of publicity associated with the materials shall be exclusively owned by Brain Master.

Furthermore, by signing below, you release and discharge Brain Master Inc., its staff, affiliates, and legal representatives from any future claims or liabilities related to the use of the materials. This includes any claim based on invasion of privacy, defamation, or misappropriation arising from their lawful use as described.

Please note that no monetary compensation will be provided in exchange for this consent. Kindly return the completed form to your Brain Master Instructor. Your understanding and support are greatly appreciated.
